The Case Manager (CM) is accountable for proactive coordination and timely transition of assigned patients to the most appropriate level of care along the health care continuum. This position is responsible for the assessment of patients and families for identification of discharge needs to arrangement of include community resources, durable medical equipment, or post-acute care services such as home health, hospice, or skilled nursing placement, to ensure the optimum use of available resources for the patients recovery and successful healthy living environment. The CM works collaboratively with physicians, nurses, members of the multidisciplinary team, and post-acute care providers in the community. Early intervention of the discharge planning process is essential to ensure the efficient utilization of resources, reduction of opportunity days and costs, while providing quality patient care. This position impacts key results, such as performance in length of stay, cost efficient resource utilization, and prevention of readmissions and unnecessary emergency room visits. Uses discretion, independent judgment, creative thinking and problem solving. Responsible for admission, concurrent and retrospective medical review of all patient populations for documentation of appropriateness of admission and continued stay. This includes the tracking and trending of days saved, avoidable days, and initiation of interventions to reduce and eliminate avoidable days; works in collaboration with interdisciplinary team members internal and external to the organization; participates in process improvement and evaluation process related to the management of patient care and resource utilization.
Diploma or associates degree from an accredited Registered Nursing program. BSN preferred
Minimum of 2 years experience in acute-care hospital or related setting; previous utilization review or case management experience.
Knowledge of private insurance, Medicare criteria for hospitalization, understanding of healthcare financial systems, benchmarking practices; Experience in facilitating change through interdisciplinary collaboration; computer skills.
