Weekend Discharge Planner / RN Discharge Planner
The weekend discharge planner is responsible for the assessment of discharge planning needs as well as the coordination of weekend discharges. This will include coordinating community resources to ensure the successful transition from hospital to home or other post acute care to ensure the patients recovery and successful healthy living environment. This includes timely intervention of the discharge planning process in order to promote the efficient utilization of resources while providing quality patient care. Uses discretion, independent judgment, creative thinking and problem solving initiative.
Bachelor Degree in Social Work, Sociology, Psychology, or related field; Masters preferred. RN may be considered with discharge planning experience
Minimum 2 years experience in a social work setting in healthcare environment.
Familiarity with local community resources and agencies, to include medical equipment companies, Nursing Homes, Home Health companies. Understanding of healthcare payor guidelines; computer skills.
