The Coordinator/Scheduler Hospice, under the direction of the Hospice Manager, is responsible for administrative functions that assist in the daily clinical team operations and clerical support to the Manager. Facilitates and organizes the scheduling of patient visits and staff assignments. Maintains accurate schedules for all disciplines within the medical record. Responsible for the oversight of agency office processes that includes: assisting with and triaging phone calls from patients, field staff, physician offices, and call center; faxing; ordering DME and supplies; contacting patients referred to Hospice; monitors NTUC and Hospice Talk Log and facilitates visits; monitors and tracks productivity as assigned. Maintains the on-call schedules of the staff and communicates schedules accurately and timely. Completes other duties as assigned. High school Diploma/GED and Certification/Registration.
