JOB SUMMARY OF RESPONSIBILITIES:
The purpose of the Case Manager Supervisor position is to work within a defined workgroup to promote the achievement of optimal clinical and resource outcomes.
Responsible for facilitating appropriate lengths of stay and reimbursement for all hospital admissions in accordance with its goals and objectives.
Acts as the key information and education resource for the interdisciplinary team.
Works to develop organization-wide approaches to problem solving.
Use variance data to identify opportunities for improvement.
Works to promote quality of care through collaboration with all service team members, patients and families.
Responsible for supervision of designated group of case managers
The Case Manager Supervisor is accountable for a designated patient caseload that is considered more complex and resource intensive. The Case Manager Supervisor plans effectively in order to meet patient needs, manage the length of stay, and promote efficient utilization of resources. Specific functions within this role include:
Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement
Application of process improvement methodologies in evaluating outcomes of care
Coordinating communication with physicians.

MINIMAL JOB QUALIFICATIONS:
EDUCATION, SPECIALIZATION/CERTIFICATION REQUIREMENTS:
Masters degree in Nursing or related health field preferred
Bachelors of Science in Nursing and Current Case Management Certification Required
Licensed RN in the state of South Carolina required
5 or more years of recent acute care hospital experience required.
5 or more years of Case Management experience required
1-2 years data entry/computer experience required.
